Not because people don't know better.
Because the system they return to hasn't changed.

If the right way is harder, slower, or unsupported, it won't hold.
More training won't fix that.
Training was completed, and employees could explain the procedures.
But, in real working conditions, behaviours weren't consistent.
Why? Because training didn't reflect real-world pressures and decision-making.
Training set clear expectations.
But, workers and supervisors reinforced something different on the job.
What happened? Behaviour followed what was reinforced, not what was trained.
